Spa Survey

Greetings!
I am a student at the University of Ottawa, in Ottawa, Canada currently
enrolled in a "Small Business Management / Entrepreneurship" course. As part
of the course requirements, I am required to create a business proposal. For
my business proposal I have elected to open a men's only spa. As part of the
marketing process, I am attempting to collect survey data from both men and
women outlining their demographic traits and their attitudes and opinions
towards spas and beauty / health care products.
All information will be kept entirely confidential and will be used only for
the purposes of this class.
